Vacancy: 04 (Mobile Health Unit- 03, RBSK- 01)
Appointment Nature & Job Location: Recruitee will be appointed on contractual basis for a period of 11 months at any PHC, UHWC as per NHM norms. Further extension may be granted based on satisfactory performance.
Mode of Selection: Walk in Interview.
Age Limit: As per Odisha NHM Norms.
Educational Qualification: Completion of Higher Secondary followed by Certificate Course in ANM from recognized institute. Registration & Live License from State Nursing Council is mandatory.
Salary: As per NHM Norms.

Interested individuals shall download and fillup applications in a given format. Attach a self attested photocopies of below documents
1. Aadhar Card/PAN Card
2. 10th & + 2 Certificate
3. Community Certificate
4. ANM Course Completion Certificate
5. Experience details as on date of advertisement to till date as prescribed
6. No Objection Certificate cum Continuation Certificate from Concerned CDM & PHO
7. First Offer Letter
